Mupirocin is a topical antibiotic primarily used for the treatment of skin and soft tissue infections caused by Gram-positive bacteria.


Basic Properties

Source and Classification: It is a metabolic compound produced from Pseudomonas fluorescens culture medium, belonging to a novel class of topical antibiotics.

Physical properties: At room temperature, it is a white to brownish solid, slightly soluble in water, and readily soluble in organic solvents such as dimethyl sulfoxide (DMSO) or methanol.

Pharmacological mechanism: Its antibacterial effect is achieved by reversibly binding to isoleucine transfer RNA synthetase within bacteria, preventing the incorporation of isoleucine and thereby terminating the synthesis of isoleucine-containing proteins in bacteria, leading to bacterial death.

Antimicrobial spectrum: exhibits high antibacterial activity against various Gram-positive cocci associated with skin infections (particularly Staphylococcus species, including drug-resistant Staphylococcus aureus) as well as pyogenic streptococci.

Skin and soft tissue infections: Widely used clinically for the treatment of primary skin infections (such as impetigo, folliculitis, furuncles, etc.) as well as secondary skin infections associated with eczema, trauma, ulcers, or small-area burns.

Nasal colonization eradication: Specific nasal formulations can be used to eliminate Staphylococcus aureus colonization in the nasal cavity, commonly employed for preventing bacterial transmission in healthcare settings or community settings.


Key Considerations

Topical application restrictions: This drug primarily exerts localized effects on small areas of skin surface, with extremely low systemic absorption rates.

Contraindications for special matrices: Ointments formulated with polyethylene glycol as the base are strictly contraindicated for large-area burns or open wounds to prevent excessive absorption of polyethylene glycol, which may lead to renal impairment; in such cases, a petroleum jelly-based matrix should be used instead.

Risk of drug resistance: Although the incidence of drug-resistant variants in vitro is very low, prolonged or inappropriate use should still be avoided to prevent inducing bacterial resistance.
